New: 2018-19 Survey Information 

District 58 invites parents to take its annual Parent School Environment Survey now through Friday, Dec. 21. This anonymous survey only takes parents a few minutes to complete, and the results will help guide the work of principals, teachers and district administrators. If parents would like to provide additional input, they may also take the State’s 5Essentials parent survey, now through Friday, Feb. 15. In addition, District 58 will administer the State’s 5Essentials Survey for students (in grades 4-8) and teachers sometime between Dec. 11 and Feb. 15. Survey results will become part of the 2019 Illinois State Report Card for your school.

All responses are anonymous. If you do not want your child to participate in the 5Essentials Survey, please sign and return the consent form to your child’s teacher by Nov. 14.

El Distrito 58 invita a los padres a participar en su encuesta anual sobre el entorno escolar de los padres hasta el viernes 21 de diciembre. Esta encuesta anónima solo toma unos minutos para que los padres completen, y los resultados ayudarán a guiar el trabajo de los directores, maestros y administradores del distrito. Si los padres desean aportar información adicional, también pueden participar en la encuesta de padres de 5Essentials del estado, hasta el viernes 18 de enero.

Además, el Distrito 58 administrará la Encuesta 5Esenciales del estado para los estudiantes (en los grados 4-8) y los maestros entre el 15 de noviembre y el 18 de enero.   • E-Pay – Use E-pay to pay for preschool tuition and summer school fees.
  • PushCoin – Use Pushcoin to pay student fees and middle school lunch accounts
    • Note: Middle school students may purchase lunch in the cafeteria using cash, or by showing their student ID cards, which will be linked to the individual student’s PushCoin wallet.  To add funds to your student wallet/lunch account, click on the link above, and access your account or create an account.  Once your account is created you can add your student by clicking Users and Add User.  From there follow the directions on how to add a student.
    • Student accounts can also be funded by sending a check made out to District 58 to be deposited at the register.

Health Services

View information related to immunizations, health examinations, sports physicals and health resources.

Student Accident Insurance Information: Parents may enroll for this optional insurance online at www.k12specialmarkets.com. This website includes detailed information regarding coverages, benefits, premiums and claim forms. Parents will have the option to directly enroll their child using a credit or debit card, or they may print out an enrollment form and mail it with a check or money order.

Before and After School Care 

Champions Program: Your right-at-school alternative to child care. For more information or to register, visit www.discoverchampions.com. All District 58 elementary schools offer before and after school care via the Champions program.
